If you choose a customized jacket/vest for an additional $5, your name will be embroidered in white on the right side of the jacket/vest. If you choose this option, please type the name EXACTLY the way you want it on the jacket/vest. For example, you may want "My Name," "My Name, RN," or "My Name, RN, SRNA." 

The jacket/vest will be embroidered with exactly what you type in the text box, so please double-check for errors.  If you want the right side of the jacket/vest to be blank, do not choose a customized option.
