Embroidery & Customization

• Left Chest: Choose "Department of Anesthesia," your name and credentials, unit name or specialty, or your organization's approved logo.

• Right Chest: Add your name and credentials (if selecting Department of Anesthesia, unit name or specialty, or logo) for an additional $7.

• Custom Logos: A minimum of 12 items featuring the same logo is required. ​​

Personalization Instructions

If you select additional right-chest embroidery (+$7), enter your name and credentials EXACTLY as you want them embroidered.

• Anna Stasia

• Anna Stasia, RN

• Anna Stasia, RN, SRNA

• Anna Stasia, DNP, CRNA

Your jacket or vest will be embroidered exactly as entered in the personalization text box. Please double-check all spelling, punctuation, and credentials before submitting your order.

If you do not want additional right-chest embroidery, select the option without personalization and leave the text box blank.

Colors, Styles & Sizing

• Multiple jacket and vest styles and colors are available in men's and women's sizes.

• Sizes generally range from XS–XL, with additional charges for extended sizes:

  • 2XL: +$2

  • 3XL: +$3

  • 4XL: +$4

  • 5XL: +$5

Pricing & Delivery

• UTHSC students and faculty have until November 10 to place an order.

• All other facilities, units, and specialties have no deadline; only a 12 minimum order for a logo.

• All listed prices include standard left-chest embroidery and delivery within 50 miles of Memphis, Tennessee.

• Orders outside the 50-mile radius will incur additional shipping charges. Please email msnafoundation@gmail.com to get shipping costs.

• Orders will be combined and submitted in bulk. Different styles, colors, and sizes may be ordered together, provided the minimum order requirements are met.

• Orders within the Memphis delivery area will be delivered to a designated location for distribution.
